Your Health Information Rights Under HIPAA

Introduction

Mind Ease US LLC and the independent licensed healthcare providers working with us ("we," "our," or "us") are committed to protecting the privacy of your protected health information (PHI). This Notice of Privacy Practices explains how we may use and disclose your PHI, your privacy rights, and our legal duties under the Health Insurance Portability and Accountability Act (HIPAA).

Mind Ease provides healthcare-related services only to individuals located in the United States, including residents of California and all other U.S. states and the District of Columbia. This notice applies to PHI collected in connection with services provided within the United States.

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of this notice as long as it remains in effect.

Uses and Disclosures of Your PHI

We may use and disclose your PHI for the following purposes:

  • Treatment: To provide, coordinate, or manage your healthcare and related services, including consultations with licensed providers and pharmacies that fill your prescriptions.
  • Payment: To obtain payment for services provided to you, including billing and collection activities.
  • Healthcare operations: To support our business activities, such as quality assessment, staff training, and customer service.

We may also use or disclose your PHI without your authorization as required or permitted by law, including:

  • As required by law, including public health and safety activities
  • To prevent or avoid a serious threat to health or safety
  • For health oversight activities
  • In response to court or administrative orders, subpoenas, or other legal processes
  • For law enforcement purposes, as permitted by law
  • To workers' compensation or similar programs

Your Rights Regarding Your PHI

You have the following rights with respect to your PHI:

  • Right to access: You may request a copy of your PHI in our records.
  • Right to amendment: You may request that we correct inaccurate or incomplete PHI.
  • Right to an accounting of disclosures: You may request a list of certain disclosures we have made of your PHI.
  • Right to request restrictions: You may ask us to limit how we use or disclose your PHI. We are not required to agree to every request.
  • Right to confidential communications: You may request that we contact you at a specific address or by a specific method.
  • Right to a copy: You may request a copy of this notice at any time. We will provide it electronically by default; a paper copy is available upon request.

How to Exercise Your Rights

To exercise any of these rights, please contact our Privacy Officer at privacy@mindease.us. We will respond to your request within the timeframes required by HIPAA.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services (HHS) Office for Civil Rights.

To file a complaint with Mind Ease, contact our Privacy Officer at privacy@mindease.us. To file a complaint with HHS, visit https://ocrportal.hhs.gov/ocr/portal/lobby.jsf or call 1-800-368-1019. We will not retaliate against you for filing a complaint.

Changes to This Notice

We reserve the right to change the terms of this notice at any time. Any revised notice will be posted on this page with a new effective date and will apply to all PHI we maintain.
